To compile its 2026 rankings, U.S. News & World Report evaluated nearly 15,000 nursing homes nationwide, assessing care for both short-term rehabilitation residents who may be recovering from a hospital stay and long-term residents with chronic needs. The Overall rating ranges from 1 to 5 and is based on publicly available data from sources including the CMS Nursing Homes Data Archive and the Long Term Community Care Coalition. Short-term and long-term care scores are weighted equally, with High Performing reflecting the highest possible score of 5. Facilities earning this distinction rank among the nation’s top providers of skilled nursing care, excelling in areas such as safety, staffing, clinical outcomes, infection control, and responsible use of medications.
