  • FSA Enrollment Form - Complete this form if you would like to enroll in Vi’s Health Care and/or or Dependent Care Flexible Spending Accounts.
  • Legal Assistance Enrollment Form- Complete this form if you would like to enroll in Vi's legal assistance benefit program.
  • Pet Insurance Salary Reduction Form - Employees are required to apply for pet insurance before coverage can take effect and before deductions for coverage begin.  Once the pet is approved for coverage, the insurance company will notify Human Resources and the employee of the coverage effective date and the monthly premium amount.  Employees are encouraged to apply for coverage online at www.petinsurance.com/viliving.  Complete this form once you've been approved for pet insurance coverage.
  • 401(k) Enrollment - Please enroll in Vi's 401(k) plan online at www.wellsfargo.com.
